Position Summary
The Provider Lead at Our House Inc. offers individualized, purposeful, and meaningful programming while serving as a frontline leader for the Day Program. In addition to meeting all requirements of a Skills & Activities Provider, the Lead acts as a resource for floor staff, offering hands-on clinical assistance and operational support. The Lead enriches, teaches, and grow the individuals in their care while setting a standard of excellence, goodwill, and professional service for staff, families, and the community. The Provider Lead is a representative of Our House Inc. and is expected to professionally represent the organization’s mission and purpose.

Pay: $0.50/hour higher than current pay rate.
Hours: 9am-2pm, with potential for additional hours during the week.
Expectations: Currently meeting expectations of Skills & Activities Provider/Mentor, following dress code, and submitting daily documentation in a timely manner.

Responsibilities
- Responsible for all aspects of Provider/Mentor position, including but not limited to: daily activities, welfare, progress, supervision, and learning experiences of individuals receiving services.
- Act as primary and initial floor support for individuals with complex needs, including performing G-tube feedings and changes (as trained) and responding immediately to behavioral challenges to ensure the safety of all individuals receiving services.
- Assist with the development and management of the program calendar and daily activities, ensuring programming remains purposeful and individualized.
- Serve as a resource for coworkers, providing real-time guidance and embodying the organization’s mission and purpose.
- Implement and provide data tracking for behavior plans, daily goal tracking, and elimination tracking.
- Provide hands-on assistance with activities of daily living, including but not limited to: hygiene, eating, toileting, changing, cleaning face, brushing, teeth, etc.
- Assist in creating meaningful programming based on individuals in services’ interests, needs, and identified goals.
- Responsible for and expected to be a positive embodiment of Our House Inc. in all interactions with individuals in services, parents, caregivers, coworkers, school district representatives, and agency representatives.
- Promptly document and report all incidents within the required timeframe.
- Document daily services, activities, and progress towards goals; ensure all paperwork is completed accurately and submitted within the required timeframe.
- Maintain a clean and presentable work environment by picking up after self and individuals in services; also taking initiative to address areas within the facility that need cleaning.
- Maintain a neat and presentable appearance, adhering to Our House dress code.
- Attend and participate in all required trainings and meetings.
- Respect and safeguard the rights of individuals in services and maintain appropriate supervision.
- Other duties as needed.
Requirements
- Must be QMAP or willing to get it within 60 days and comfortable passing medications.
- Must be willing to get g-tube trained and be willing to perform regularly.
- Must take drug screen and pass.
- Must be willing to take and pass background checks.
- Must be in good physical, mental and emotional health.
- Must be able to lift a minimum of 50lbs; if required.
- Must have current CPR and first-aid certification.
- Must be 18 years of age or older.
- Must be dependable, reliable, and punctual.

- Must have good critical thinking and problem-solving skills.
- Must have good interpersonal and communication skills.
- Must have ability to remain calm and provide leadership during behavioral crises. dsd
